Something must be up with my install then. I've tried the same settings as you and it still fails. I'll try to uninstall and reinstall or try a portable version.

I've been testing some more and found that if I change the "When output files already exist" value to rename instead of replace, the conversion works on all pages. That's not good because I need to replace files in a whole lot of directories, not make duplicates of them.

At a quick look, hopefully not too quick, I don't have a problem with 'Replace' either:
The resulting multipage TIFF opens and views normally...

I'm trying to start from scratch, but XnConvert is storing settings somewhere. Every version I try is holding all of the settings I previously. How do I clear that out and start fresh? I've checked the registry and found nothing.

Do you have the problem even with a simple 'Rotate' action, which is how I'm testing?
I can only suggest that you upload screenshots of your tabs, at least the Input, Output, Status and Settings tabs, although I don't have any ideas about what might be causing the problem.

Yes, I get the same error on a rotate. If I get some time I'll upload screenshots, but they're no different than what I've described.

I wonder what the crap is wrong with my install then? Is there a way to clear out all settings and existence of xnconvert and try again?

I don't see it as a problem with your install, although I might be wrong, and in any case you reported the same result on another computer, rather as resulting from a setting you're using, although it is hard to think what could cause the results you are seeing.
I would again suggest uploading screenshots of your tabs mentioned above...
