Fred, I think maybe "perfect" is a little too ambitious. How about we go with "most excellently designed," and assume--us being human and all--that perfection of either design or implementation is a little beyond our reach.
Of course, your two number one objectives are worthy, and the best programs do exactly that. But I think that even programs which fall short--even very short--of those goals can still benefit hugely from using "pessimistic programming" techniques. The good feedback and clear program-to-user communications in the case of operation failure can overcome a lot of situations/issues/things that otherwise would be problems.
Maybe you should write a book so we can get rich and famous.
