Do you have always this problem when editing IPTC on multiple JPEG files? and on one file?
If it fails on a selection of files, if you try again, same problem (after removing .bak)?

The quantity of files seems to have no bearing; it will sometimes do it with only a single file and sometimes with many.
Usually, after I remove .bak and try again, it will work fine.
Often, when it creates a .bak, my edit will be to on the .bak and not the original I intended.

indicated by Gérard 91?
@BuckSkin: Have you considered checking access rights with a utility like
NTFS Permissions Tools or
LockHunter?

How many databases does XnViewMP utilize/have; what are they called and where are they located ?
Seeing as all metadata is written into the files, I should lose nothing by deleting databases and letting XnViewMP create them anew, right ?
I would be all for purging the program of all databases and starting fresh to see if that will cure this .bak file problem.

The location depends on your configuration in
Tools →
Settings… →
Integration →
Paths.

Tools →
Settings… →
Metadata →
IPTC & XMP?
For now, I recommend renaming
Thumb.db and
XnView.db to
Thumb.db.old and
XnView.db.old.

You can tell XnView to
check integrity of catalog at startup or disable cataloging altogether in
Tools →
Settings… →
Catalog →
Catalog.

I am fairly certain these settings are default.
I want XViewMP to write IPTC, XMP, Comment, and GPS into the files metadata.
I would prefer it not alter my keyword tags at all.
Do you recommend any changes to what you see here ?
Catalog was enabled and I disabled it.

So you need to disable 'Export Catalog category...'
About .bak problem, is it possible to send you a test version to understand the problem?
