Settings
Metadata > IPTC&XMP tab > Categories and keywords group

When 'Read hierarchical keywords from images as categories' checkbox is unticked one of its child checkboxes remain active

Thanks to your detailed description I can reproduce the problem.

This problem is supposed to be fixed in XnView MP 1.4.1. Please check and confirm the bug fix here.
