B2: Minor: Batch rename - initial focus

In batch rename, initial focus is on 'Rename' button (OK) and on 'up' button - it should be on 'Name template' (name field), I think.
If focus is on 'Name template field' and tab key is pressed, focus is switched to 'duplicate' button - instead of 'insert' button.

Fixed, but - minor - if Name templ. is disabled, focus is still on 'up' button, it should be on "[ ] Name template", so it could be enabled with <space>, IMHO.
And:

Not fixed.
It should be:
- [ ] Name template
- area below [ . . . . . v ]
- Insert
- Duplicate
Or even:
- [ ] Name template
- area below [ . . . . . v ]
- Start
- Insert
- Step
- Duplicate
