File already exists: skip all

A new button to 'skip all' files would make updating another directory faster when you don't actually want to examine each matching image.
Copying and pasting using <ctrl + c> and <ctrl +v> using Xnview is not good for updating directories. It automatically creates copy of each file without asking to skip any.

I fully agree. But I think we need also Replace all and Rename all.
So I suggest this option:
[x] Apply to all files [Replace] [Rename] [Skip] [Cancel]

Yes, "thumbnail dialog" should be shown here too IMO.

"[x] Aplly to all files" would be a good and common solution. You have my support.

And what do you think, it should be always saved, or always disabled, or saved and then disabled at next xnview startup?

Not sure that 'Apply to all' is good, but have not the solution

I agree except I do not think that Cancel would need to apply to all because it would stop the copy operation.
