I was curious about the permanence of favorites and categories, so I tried renaming and moving files around folders; none updated. I'm hoping that it could process the file's categories when renamed/moved to keep in sync, and process a folder's favorite status. When doing such administration from outside xnview, I doubt there's any elegant way to work it.

Haven't tried B4 yet, will soon. (Although it seems to be more unstable than usual for most.)

Yes, currently there is no synchro between folder on disk, and folder in the database.
