Go to Tools » Options... » Browser » File List » Misc and de-activate Recognize only by extension.

To be able to read dicom picture, file must have .dicom or .dic extension
Using XnView for DICOM Images with "wrong" extensi

I am using DICOM Images with ".IMA" extension (created by Siemens-Syngo) - When the first file in Folder is renamed to ".DCM" the viewer of xnview can display all other files although their extension is still ".IMA" (why?).
Is there a workarround to assign the ".IMA" extension to DICOM files?

No, dicom files must have dcm extension to limit crash...
