Questions about Categories

Why not directly this in the Categories Filter tab?

Does anyone know the proper semantics for creating a category list using txt or csv format? I know that it involves creating the parent/subfolder relationship using Tab. But I think there are other formatting tricks. Seems like I saw an example online once, but I can't find it now.

Any help would be appreciated.
