When left-clicking an image, it zooms to 100% with pan, but zooms back to "fit image to window" when the mouse button is released. What setting do I change to make it stay zoomed to 100% while letting go of the mouse button, then allowing the image to be panned?

Welcome to the forums...
You can however hit the * key (which is the default shortcut for cmd_zoom100, as set in Tools->Settings:Interface:Shortcuts (Viewer)) first, and that will zoom to 100%. And then you can execute a pan. Or you could zoom with the mouse wheel first, if you set that option.
You can switch between "Zoom & Pan" and simply "Pan" (or assign them to different buttons, modified or not) in Settings:Interface:Mouse. Zooming with the mouse wheel is also configured here.
If you want Zoom 100% (cmd_zoom100) assigned to a mouse button, that is possible by assigning the keyboard shortcut to a mouse button using the excellent X-Mouse Button Control (XMBC) software, on Windows at least.
