Generate a list of the category tree

How can I generate a list of the category tree I created? I'd also like to get a list that enumerates the folder and file names I assigned to each category.

There's a command in Create / File List that's supposed to generate a CSV, XML, or TXT file. I must be doing something wrong because the generated CSV only has one item: DirectoryFilename With Ext. In other words, it doesn't generate a result; it just repeats the criteria I chose for the list. Any help would be greatly appreciated.
