When a thumbnail is created, it will be first displayed without EXIF orientation (it will be correctly displayed once you re-enter the folder, though).
Edit: it also happens when you 'Edit IPTC data' and cancel -> thumbnails will be shown without EXIF orientation just afterwards.
A3: thumbnails are not EXIF oriented on creation
