I've now tested the new DLL on Windows XP SP3 and 2K SP4 (after removing the settings of older versions from the registry).
I still get five zero-length TMPs per MP3 file

What about %WinDir%\TEMP\?

Any MP3 will do, as long as it does *not* have an image embedded in it.

Indeed.

With the shell extension from beta 4, I get 3 empty temp files in my %temp% directory when right-clicking MP3 files that doesn't contain an embedded picture.
