The width is displayed with "15.00 cm", but the real size on screen is "12.50" (measured with a ruler)
The physical size refers, if ever, to printing, not the screen

But I don't get any physical size when saving a shot with XNVIEW ... how did you do ?
Then I imported the PNG to MS Word 2003, display Word with "100 %":
- The image properties are 100%, 15 cm.
- The ruler in Word shows 15 cm too; the ruler in my hand shows 17 cm.
- But the display is not sharp, and displaying "100% in XnView" and "100% in Word" side by side shows a remarkable difference.
What happens here?
I can't reproduce any bug in XN. Most likely this is due to World's size understanding and related to the
pHYs chunk in PNG format. Does you file have it ? What pixel per inch or pixel per meter value ? Try to remove it with PNGOUT (XN/NC can't ???).
BTW, PNG is a pixel based image format so the only "real" size is the size in pixels, the printing size can be any ...
